About letter of introduction

First time visitor

A letter of introduction is required at the first visit to our hospital.

Please make an appointment online or by fax from your family doctor in advance, and bring the referral letter with you when you visit. If you make an appointment in advance, the waiting time will be less. Please note that if you do not have a referral letter, you will be required to pay the unintroduced patient addition.

Due to the revision of the Health Insurance Law in April 2016, "first-time patients without a referral letter from another medical institution" and "revisit patients who have been referred by a doctor to another medical institution" "Those who have continued to visit our hospital despite having made a request to receive medical treatment," were obliged to pay a certain amount or more. Our hospital is a target hospital for this system, and we are responsible for the following amounts. Thank you for your understanding and cooperation.

Non-referral patient addition
If you do not have a letter of introduction at the time of your first visit, you will be required to pay a separate fee (own expense) in addition to the usual medical fees.

Notes

  • First and second visits are medically determined by a doctor.
    (Please note that if there is a gap between your last visit and if you are not seen on the scheduled date, you may be treated as a first visit even if you have the same disease.)
  • Medical and dental services are treated separately for insurance purposes, so you will need to pay for each.
  • Even if you visit other departments of our hospital, you may be charged for the first visit.
  • If you bring a letter of introduction from another medical institution, if you need to be transported by ambulance, or if you have a medical examination on a holiday or at night, etc., there is no need to pay.

Outpatient waiting situation

This is a system that allows patients to check the progress of medical examinations from outside the hospital using smartphones, etc., in order to improve convenience for patients. In addition to checking from our website, you can also check from the QR code printed on the reception table (information) at the time of reception for return visits.

How to use the ringer

  1. A pager will be handed to you at the return visit reception desk or the first visit reception desk.
  2. This display number will be the call number for today. This number will be displayed on the information display board in front of the examination room.
  3. By pressing the button, you can switch the screen and check the schedule for the day.
  4. We'll let you know when it's time to see the doctor. Be sure to press the button and move to the waiting room.
  5. You will be able to enter the examination room soon, so please be sure to press the button and wait in the examination room seat.

About Online Eligibility Verification

At our clinic, we have introduced the online qualification confirmation promoted by ministry of Health, Labor and Welfare notation. We strive to provide high-quality medical care by acquiring and utilizing medical information. In order to obtain and utilize accurate information, we ask for your cooperation in using the Minor Health Insurance Card.

It is to check medical insurance qualification information, drug information, specific medical examination information, etc. online using the IC chip of the My Number card or the code number of the health insurance card.

1. My number card will be available as a health insurance card.

If you have a My Number card, you will be able to smoothly verify your medical insurance eligibility without having to present your insurance card or other documents at the counter.

2. Applying for and presenting a ceiling application certificate is no longer necessary.

If you have a My Number Card, with the consent of the patient, you will be able to obtain information such as a ceiling application certificate at our hospital without applying to an insurer. no longer need to pay at the counter.

Patients who wish to use their My Number Card as their health insurance card are requested to keep the following in mind.

1. Application for use
Before coming to our hospital, you can apply to use your My Number card as a health insurance card on the My Number Portal site, or you can apply using the card reader installed at our hospital.
2. Installation location
Our hospital has three locations: the insurance card confirmation counter on the 2nd floor of the main building, the emergency reception desk on the 1st floor of the main building, and the general reception desk on the 2nd floor of the south building.
3. Compatible functions

We support the following functions:

  2. (2) Confirmation function of ceiling application certificate
  3. (3) Ability to view specific health checkup and drug information

FAQ

As a general rule, outpatient services at our hospital are based on a referral system.

You can also make an appointment in advance via the internet or fax from your family doctor, so please use that.
In addition, if you do not have a referral letter, you will be required to pay 7,000 yen for medical and 5,000 yen (tax included) for dentistry at the time of your first visit, in addition to the insurance co-payment. .
Medical and dental services are treated as separate medical institutions for insurance purposes. Please understand that each person will be responsible for the cost.

Parking is available (for a fee), but we ask that you come to our clinic by public transportation whenever possible, as parking is limited.

From February 1, 2025, the price for patients who are not Japanese nationals and do not have valid public insurance in Japan will be 30 yen per item.

In principle, treatment for complications following cosmetic medical treatment is not covered by insurance (self-paid medical treatment).
