In order to protect the lives and health of Kobe City as a core hospital in Kobe City,
We aim to provide quality medical care.

We aim to provide medical care with a particularly high degree of severity. We ask for your understanding and cooperation regarding the following so that as many citizens as possible can use the facilities.

  1. In order to accept emergency hospitalized patients, you may be asked to move to another ward or room.
  2. If the attending physician determines that you can leave the hospital, you will be asked to leave the hospital immediately.
  3. If you are hospitalized with a referral from another medical institution, you will basically be asked to return to the original medical institution.
  4. Regarding transfer to another hospital or home care, please consult with your doctor and come to the Patient Support Center as soon as possible.
  5. As a general rule, we will leave the hospital at 10:00.

Hospitalization contact and procedures

    The hospital will contact you by phone regarding the date and time of your admission. Please see the table below for the items required for the procedure on the day of hospitalization.

    For procedures on the day, please come to the admission and discharge reception on the 2nd floor of the main building.

  3. completion

    After completing the procedure, go to the ward where you will be admitted.

Items necessary for procedures on the day of hospitalization

  • Hospitalization application copy
  • Patient registration card
  • Health insurance card or My Number insurance card
  • Information on hospitalization (booklet)
  • Recipient certificate of welfare medical expenses and other public expenses
    (If you have one)
  • High-cost medical expenses ceiling certificate (for those who have it)

  • Hospitalization fee payment form
  • Hospitalization history confirmation slip
  • Application for loan of hospital clothes (required/unnecessary)
  • Hospitalization Set Application Confirmation (only for those who have applied)
  • Consent form to enter private room with difference price
    (Only for those who enter a private room with room charge difference)

Notes

If you are unable to be admitted to the hospital on the scheduled date or time, please contact the Admissions and Discharge Reception as soon as possible. We will let you know which ward you will be admitted to on the day of your admission.

    Medications you usually use (only medicine, acupuncture medicine, eye drops, self-injection at home, etc.), medicine notebook, medicine instructions, pacemaker notebook

    Soap, shampoo, conditioner, toothbrush, toothpaste, cup, denture case, detergent, shaver, brush, towel, bath towel, etc.

  • daily goods

    Tissue paper, wet wipes, waste plastic (about 40cm x 40cm), nail clippers, writing utensils, earphones for watching TV, etc.

    *Hospital-designated nightwear will be provided (charge applies: 110 yen/day (tax included)), but please let us know if you would like to use your own nightwear.

  • overcoat

    Indoor footwear (To prevent falls, comfortable shoes are safer.)

  • Non-woven mask

    Please prepare as many as you need during your stay.

  • *Chopsticks, spoons, and cups will be served at the meal. If you need anything else, please contact us individually.
  • *If you receive a prescription for medicine at another facility while you are hospitalized, you may not be able to use your health insurance card (you will have to pay the full amount yourself), so please consult with your doctor before visiting the hospital.
  • *If you have a reservation ticket for outpatient or examination, please bring it with you.
  • *For cash and valuables, please be sure to put it in the safety box (simple safe) on the floor next to each bed and lock it. Correct, don't carry more cash than you need.
  • *At our hospital, we have a service where you can use diapers, towels, toothbrushes, etc. on a daily basis.

  • *The Director 's rounds and surgery date will be explained at the time of admission.
  • *Meal times may vary by about 15 minutes.

To enter the ward, you will need the "security card key" that you will receive during the admission procedure. Be sure to carry it with you when you leave the ward. The patient's card key has a prepaid card function. If you want to use the refrigerator, TV, or internet in the hospital room, please charge the card with the necessary amount.

Our clinic offers a service that allows you to use diapers, towels, toothbrushes, etc. on a daily basis.

The main document fees are as follows.

  • Certificate of application of the Automobile Liability Compensation Security Law (5,500 yen including tax) / Compulsory automobile liability insurance medical fee statement
  • Various certificates (doctor opinion required) (3,300 yen including tax) / pregnancy certificate, birth certificate, etc.
  • Various certificates (no doctor opinion required) (2,200 yen including tax) / Medical expenses receipt certificate
  • Medical certificate (to be submitted to insurance companies, etc.) (5,500 yen including tax) /
    Insurance company (hospitalization/surgery) certificate, pension, physically disabled, etc.
  • Medical certificate (other than the above) (3,300 yen including tax) / Specific disease (clinical investigation individual card), medical examination certificate, etc.

Parking is available (for a fee), but we ask that you come to our clinic by public transportation whenever possible, as parking is limited.

The parking fee is 160 yen/hour.
