  • The patient requests to see the Outpatient Care for heart failure at the Cardiology department.
  • If instructed by doctor, your nurse or outpatient clerk will make an appointment for you at the heart failure Outpatient Care.

Target patient Patients with heart failure who are currently receiving outpatient care at our Cardiology department
  1. Patients with chronic heart failure are increasingly being hospitalized
  2. Patients diagnosed with heart disease for the first time
  3. Patients who are anxious after defibrillation or biventricular pacemaker use, or due to arrhythmia
  4. Patients who have problems with their medical care and wish to consult with a nurse
  5. Patients struggling to balance treatment and work
content
  1. We listen to symptoms of heart failure and work to control and alleviate them.
  2. We will explain the monitoring methods necessary to quickly detect worsening heart failure.
  3. We will check your living and activity status at home and provide advice on medication, fluid and salt management, and exercise methods.
  4. When necessary, we will provide advice, taking into consideration your family's situation.
  5. We will strive to reduce anxiety regarding worsening heart failure and defibrillation procedures.
  6. We will listen to any problems or concerns you may have in your daily life and work together to think of ways to live a heart-healthy life.
  7. After doctor has informed you of your condition or if you are considering new treatment, we will work together to think about what to do next.
  8. We will ask about your employment situation and work together to consider how you can balance work and family life in line with your physical condition.

Actual flow of Outpatient Care

*If you have tests such as blood tests, X-rays, or electrocardiograms, please take these tests before Outpatient Care.

  1. Weigh yourself to see if you are gaining weight.
  2. Measures blood pressure, pulse and oxygen saturation.
  3. Check for shortness of breath, palpitations, and swelling in the legs. Listen to breathing sounds and feel the abdomen.
  4. If there have been any X-rays or other tests, we will review the results together. If things have gotten worse, we will look back on what may have caused the condition to worsen.
  5. We will listen to your life at home. We will think together about what you should be careful of in your daily life so that you do not have to be hospitalized due to heart failure.
  6. We will discuss ways to prevent heart failure together. If necessary, we will provide you with a heart failure notebook and ask you to record your daily blood pressure and weight.
  7. The contents of the diary you have written at home will be checked at the next heart failure Outpatient Care.
  8. Depending on your physical condition, we may ask you about how to deal with your illness in the future.
