The Central Operating Department is located on the 4th floor and has 19 operating rooms (2 hybrid rooms, 3 clean rooms, and 1 negative pressure operating room) equipped with advanced medical equipment. It is suitable for a wide variety of surgeries.
The hybrid operating room is an operating room where angiography can be performed at the same time as Neurosurgery. It is used for craniotomy surgery that requires angiography. Clean rooms are used for surgeries that require a high degree of cleanliness, such as Orthopedic Surgery artificial joint surgeries and Cardiovascular Surgery surgeries. We also have 3 surgical support robots (2 da Vinci, 1 hinotori), which are mainly used in Urology, Obstetrics and Gynecology and gynecology, and surgery. The negative pressure operating room is an operating room for infectious diseases, and was mainly used as an operating room for COVID-19 positive patients during the COVID-19 epidemic.
In addition, a CT imaging device is installed in the operating room, so for patients who need CT imaging immediately after surgery, CT imaging can be performed immediately without having to go to the radiology department on another floor. Because it can be done, safety and speed are compatible. In addition, the operating room is equipped with a post doctor anesthetic recovery room (PACU), where nurses and Anesthesiology conduct thorough observations before returning to the general ward after surgery.
The number of surgeries has increased year by year, reaching more than 10,000 in 2019 (the majority of Ophthalmology surgeries are not included in the number because they are performed at the adjacent Eye Center Hospital). After that, the number of cases decreased slightly due to the COVID-19 epidemic, but now it is on the rise again.
Intensive Care Department/Critical Care Department
The Intensive Care Unit consists of the 8-bed EICU and 6-bed CCU in the Critical Care Center on the 1st floor, and the 8-bed GICU adjacent to the Central Operation Department on the 4th floor. Specialized intensive care physicians and nurses for critically ill patients who require artificial respiration, respiratory support by ECMO, circulatory support by IABP or PCPS, liver/renal support by blood purification, etc. Specialists, pharmacists, occupational therapists, nutritionists, clinical engineers, and other professionals work together to provide treatment and promote early recovery. EICU is primarily intended for emergency department-mediated severe respiratory failure, severe infections, multiple organ failure, severe trauma, post-emergency surgery, severe burns, and severe poisoning. CCU is intended for cardiovascular diseases such as acute myocardial infarction and severe heart failure. The GICU is intended for patients who have undergone major surgery such as cardiovascular surgery or surgery for esophageal cancer, as well as severely ill children. I am treating it. There are 16-bed high-care unit GHCU adjacent to the GICU, 12-bed high-care unit CHCU for cardiovascular disease in the heart center, and 12-bed stroke care unit SCU in the stroke center as departments equivalent to the intensive care unit. We are now conducting intensive observation and treatment. Of the 700 beds in the entire hospital, a total of 98 beds, including 36 emergency beds in the 1st floor critical care center, are in intensive care and critical care departments, excluding neonates. The Department of Anesthesiology and Emergency Department of our hospital conducts an "epidemiological study on artificial respiration therapy" for patients in the intensive care unit.
Internationally, there has been extensive research on mechanical ventilation and ventilator weaning, and changes in clinical practice have also been reported. However, in Japan, it has not been done so far.
Therefore, in 2014, major hospitals across the country cooperated and started research aimed at investigating the ventilator settings used in Japan and the factors that may affect the ventilator settings. We collected data from patients who entered the intensive care unit from May 1, 2015 to September 30, 2015 in our Anesthesiology and emergency department, and received artificial respiration therapy. I plan to use it for research purposes.
This research collects medical records (examination results, etc.) obtained in the daily medical care of the target patients, but there is no special treatment or examination, so there is no direct benefit or disadvantage to the patient. never occurs.
