INSTRUCTIONS FOR INPATIENTS

You might be concerned about hospitalization, and we want to do our best to make you comfortable during your stay here. Here is some important information to help you. Please read through this carefully. Questions can be answered at the Admission and Discharge Office on the first floor.

HOSPITALIZATION

  • We will inform you of the date of your admission to the hospital by telephone.
  • Please bring the following items with you:
    • Health Insurance Card, if you have one
    • Hospital I.D. Card
    • A copy of your application for hospitalization
    • The written and signed oath (the last page of this pamphlet)
    • Your personal seal (inkan/hanko), if you have one
  • The date of your admission may be changed if there are unavoidable circumstances.
  • If you want to change or cancel the date of your admission to the hospital, please call the

ITEMS THAT YOU WILL NEED WHILE IN HOSPITAL

・wash basin ・toothbrush, toothpaste and mug ・soap
・shampoo and/or other hair care products ・a box of facial tissues
・chopsticks, or knife, fork and spoon ・napkin ・dish-drying cloth
・small tea-pot ・3 bath towels ・3 hand towels ・3 face towels
・underwear ・two sets of night clothes ・slippers
・20 plastic bags ・2 clips to hold the plastic bags

VISITING

  • Visitors may ask directions to your bed/room at the Admission and Discharge Office on the first floor.
  • In the evening, visitors may ask an attendant in the guard room near the main entrance on the first floor.
  • Visiting hours are from 3:00 p.m. to 8:00 p.m. on weekdays, and also from 10:00 a.m. to 12:00 noon on Saturdays, Sundays, and national public holidays. Please register at the nurse station in the ward when visiting.
  • Permission must be obtained at the nurses' station in the ward for any visits outside of the visiting hours described above.
  • Too many visitors at one time, or visitors with children will not be allowed.
  • Depending on the condition of the patient, the hospital may refuse to allow visitors or may ask them to wait.

RULES TO BE OBSERVED AND OTHER INFORMATION

  • You are requested to follow the instructions of doctors and nurses.
  • If you want to go out of the hospital, please consult your doctor.
  • Smoking, drinking alcohol, or playing games loudly are not allowed in the hospital. Please speak quietly.
  • Please cooperate in keeping your room clean and neat.
  • All lights must be turned off by 9:00 p.m.
  • There are no facilities for the safekeeping of money or valuables. Please watch them carefully. The hospital is not responsible for any loss or damage of patients' valuables.
  • You may not use the telephone at the nurse station. Please use the pay phones provided for your use.
  • Please do not walk around in your night clothes.
  • If you want to use any electric appliances other than a portable radio or a shaver, please ask a nurse.
  • The Post Box for Mail is on the first floor. The ATM cash machine is on the second floor.
  • Shops and cafeterias are in the basement of the front section.
  • There are no laundry facilities available in the hospital.

HOSPITAL CHARGES

  • We calculate hospital charges according to Health Insurance rates.
  • The monthly bill is calculated the following month. Please pay your hospital bill at the bank on the second floor. The bank is open from 9:00 a.m. to 3:00 p.m. on weekdays.
  • If it is difficult for you to pay the bill before you are discharged, please consult the Admission and Discharge Office.
  • Please see the attached paper about room charges.

SOCIAL WORKERS

  • If you have a financial problem regarding hospital charges or problems during your stay in the hospital or after you are discharged, please feel free to consult our medical social workers. Consultations with medical social workers are held in the social workers' office on the second floor.

DISCHARGE FROM THE HOSPITAL

  • Upon your discharge from the hospital, please pay the bill at the bank and then go to the Admission and Discharge Office. You will then receive a "Bill Paid" card there. Please give this to the chief nurse in your ward before you leave.
  • The first appointment with a doctor at the Outpatient Clinic after you are discharged can be made at the Admission and Discharge Office when your bill is paid.
